DATA HIDROLIK MAKINA SANAYI A.Ş. EXPLICIT CONSENT STATEMENT FORM (B)

I acknowledge that I have read this Information Notice and, within the scope of the Law on the Protection of Personal Data No. 6698 (PDPL), I hereby give my prior consent to the collection, recording, processing, and storage of my personal data by Data Hidrolik Makina Sanayi A.Ş. in accordance with the principles set out in the law.

Pursuant to the “Law on the Protection of Personal Data No. 6698” and the “Regulation on the Processing of Personal Health Data and Ensuring Privacy”, I consent to the processing of my personal data, special categories of personal data, and health data; including all kinds of personal data, whether verbal, written, and/or provided electronically, that identify or help to identify me, by Data Hidrolik Makina Sanayi A.Ş., for the purposes of candidate selection, execution of employment contracts, fulfillment of contractual obligations, provision and planning of health services, and sharing with other persons and organizations when required by relevant legislation. I declare that I have been informed by the Company regarding these matters, that I have been informed within the framework of the PDPL, and that I give my explicit consent as described above. I accept, declare and undertake that I have given my explicit consent.

In accordance with Article 11 of the PDPL and related regulations, I acknowledge that I have the right to learn whether my personal data is processed, to request information if it has been processed, to learn the purpose of processing and whether it is used in line with this purpose, to know the third parties to whom personal data has been transferred domestically or abroad, to request correction if personal data is incomplete or inaccurate, to request deletion or destruction of data when the reasons for processing no longer exist, to request that such correction or deletion be notified to third parties to whom data has been transferred, to object to any outcome against me resulting from analysis by automated systems, and to demand compensation for any damage arising from the unlawful processing of my personal data. I acknowledge that I can exercise these rights by completing the Data Subject Application Form and submitting it to Data Hidrolik Makina Sanayi A.Ş. through the methods specified in the form, through a notary public, or by other means defined in the PDPL.

Furthermore, I declare that the personal data I have shared with the Company is accurate and up to date, and I agree to inform the Company of any changes to this information.

I give my explicit consent for the processing, use, sharing, and retention of my personal data, including special categories of personal data, limited to the purpose of processing within the relevant scope and for the necessary period of time. I confirm that I have been informed about these matters, and I declare that I have read and understood the Data Hidrolik Makina Sanayi A.Ş. Personal Data Protection Policy and this Explicit Consent Statement Form.
